Effective Date: April 7, 2024 Welcome to Glitzz & Pizazz Kidz Spa LLC! These terms and conditions govern your use of our kids spa services. By booking an appointment or using our services, you agree to comply with these terms. Please read them carefully before proceeding. Appointment Booking: - Individual services can be booked through our website, via walk-in appointments, or by phone. - Parties must be booked exclusively through our website. Age Restrictions: - Our spa services are tailored for children aged between 4 and 11 years old. - Parents or guardians must sign a consent form for children under the age of 18 before services are rendered. Health and Safety: - Children with contagious illnesses or infectious conditions should not visit the spa until they are no longer contagious. - Parents or guardians are responsible for disclosing any health conditions, allergies, or sensitivities their child may have before booking appointments. - Glitzz & Pizazz Kidz Spa LLC reserves the right to refuse service if it may compromise the health or safety of the child or others. Supervision: - Only children receiving services and their parent or guardian are allowed on the spa premises during appointments. - For parties, up to 8 adults are permitted to accompany the children. ---- Additional guests, including siblings and friends, are not permitted unless they are also receiving services and accompanied by their parent or guardian. Behavioral Expectations: - We expect all children to behave respectfully and appropriately during their spa visit. - Disruptive or disrespectful behavior towards staff or other clients may result in termination of services and removal from the premises. - Parents or guardians are liable for any damages caused by their child during their visit. Cancellation Policy: - For individual appointments, we require a minimum of 24 hours notice for cancellations or rescheduling. - For parties and group bookings, we require a minimum of 72 hours' notice for cancellations or rescheduling. Payment and Pricing: - We do not accept cash for service payments. - A $100 non-refundable deposit is required at the time of booking for parties. The remainder of the payment is due 72 hours before the scheduled party. Failure to provide adequate notice may result in forfeiture of the deposit. - For the remaining balance for parties, a payment link will be sent once we have confirmed the booking. - Service prices are listed on our website and may vary based on selected packages or add-ons. - Payment is due at the time of service for individual appointments and can be made via credit/debit cards and other electronic payment methods. - Tips are greatly appreciated. Personal Belongings: - Glitzz & Pizazz Kidz Spa LLC is not liable for any loss or damage to personal belongings brought onto the premises. - We recommend leaving valuables at home or securing them during your visit. Feedback and Concerns: - We value feedback from parents and guardians to enhance our services. - If you have any concerns during your visit, please inform our staff immediately. Changes to Terms and Conditions: -Glitzz & Pizazz Kidz Spa LLC reserves the right to amend these terms and conditions at any time without prior notice. -Parents and guardians are responsible for reviewing these terms periodically for updates. By booking an appointment with Glitzz & Pizazz Kidz Spa LLC, you acknowledge that you have read, understood, and agree to abide by these terms and conditions. If you do not agree with any part of these terms, please refrain from using our services.
